Job Opportunity: Nurse Case Manager, Emergency Department (ED)

Location: Tuba City, Arizona
Employment Type: Contract
Contract Length: 13 weeks


Facility Overview

  • Located in Northwestern Arizona, serving a 4,400 square mile area including Navajo, Hopi, and Southern Paiute reservations.
  • JCAHO accredited health center with 73 beds.
  • Provides comprehensive inpatient/outpatient emergency services, dental, ophthalmology, orthopedics, OB/GYN, oral surgery, eye surgery, and urological procedures.
  • Approximately 49% of surgeries are outpatient/ambulatory.
  • Diverse community with Native American tribes and Anglo, Hispanic, African American employees.
  • Multidisciplinary medical staff including surgeons, general medical officers, psychiatrists, pediatricians, family practice physicians, and specialists.
  • Nursing staff includes RNs, LPNs, nursing assistants, nurse practitioners, nurse midwives, CRNAs, and community health medics.

Position Summary

The Nurse Case Manager in the Emergency Department is responsible for:

  • Coordinating and maintaining processes to increase patient throughput to the appropriate level of care.
  • Collaborating with patients, families, multidisciplinary teams, physicians, community resources, and payers.
  • Assessing and managing patients and families in ED, inpatient, and outpatient settings.
  • Determining patient status using InterQual Criteria and facilitating care management including discharge planning and referrals.
  • Working closely with case managers, social workers, admissions, and leadership across departments.
  • Participating in development of clinical practice guidelines and physician order sets to improve quality and reduce costs.
  • Evaluating patients for medical necessity and care coordination needs in the ED.
  • Coordinating care for patients with abuse, chronic pain, frequent utilization, and mental health issues.
  • Interacting with patients of all ages, from newborn to geriatric.

Qualifications

Education

  • Associate’s degree in Nursing

Licenses/Certifications

  • Valid, unrestricted Professional Nursing License in any U.S. state, territory, or Puerto Rico
  • Current Basic Life Support (BLS) certification by the American Heart Association (must be maintained)
  • Case Manager Certification (CCM or ACM) required within 2 years of hire if not already obtained

Experience

  • Minimum 2 years of current care/case management experience in the Emergency Department
  • Minimum 2 years of patient care experience in hospital, home health, or skilled nursing settings
  • Proficient with Electronic Medical Records, Microsoft Outlook, Word, and Excel
  • At least 1 year of travel experience

Additional Information

  • Fingerprinting clearance required prior to start
  • Shared contractor housing available; no pets or family allowed per hospital policy
  • Orientation occurs every 2 weeks
  • COVID & Flu vaccination exemptions may be accepted after submitting a declination form
  • Housing application must be submitted 2 weeks prior to start
