<h3>Assignment Overview</h3><ul><li>Shift: Days, 5x8hrs</li><li>Hours: 40 hrs/wk</li><li>Start Date: Nov 15, 2026</li><li>Length: 13 weeks</li><li>Openings: 1</li></ul><h3>Description</h3>American Traveler is hiring an experienced Care Manager II for a hospital Case Management role requiring 2+ years of acute care case management experience and EPIC proficiency.<h3>Details</h3><ul><li>Works within the hospital's Case Management department</li><li>Manages a daily caseload of 20-25 patients in a high-pace environment</li><li>Standard schedule of 5 days per week, 8 hours per day, generally 8:00 AM to 5:00 PM</li><li>May be required to arrive at 7:30 AM and remain until all cases are completed for the day</li><li>Collaborates with physicians, nursing staff, and multidisciplinary team members across the continuum of care</li></ul><h3>Requirements</h3><ul><li>Active RN or LMSW license in the state of TX or compact required</li><li>BLS certification preferred</li><li>Case Management certification preferred</li><li>Minimum of 2 years clinical experience required, with 1 year in acute care setting preferred</li><li>Graduate of an accredited nursing program (BSN preferred) or MSW required, unless substituted by 5+ years demonstrated success in a Care Manager I role</li><li>Required experience in Case Management, utilization review, MCG criteria, and InterQual criteria</li><li>EPIC EMR experience required</li><li>Acute hospital case management experience highly preferred</li><li>Strong verbal and written communication skills with ability to interact with diverse populations</li><li>Critical thinking, analytical skills, and ability to multitask in a fast-paced environment</li></ul><h3>Additional Information</h3><ul><li>Coordinate patient progression through the continuum of care and develop individualized care plans from admission through discharge</li><li>Responsible for care coordination and discharge planning, including identifying post-acute care needs and resources</li><li>Monitor length of stay and collaborate with interdisciplinary teams to remove barriers and expedite discharge</li><li>Act as patient advocate, negotiating with payors, agencies, and vendors to coordinate resources</li><li>Participate in multidisciplinary rounds and escalate cases to Physician Advisors or CM Director as needed</li><li>Document in the medical record per regulatory and departmental guidelines</li><li>May assist with special projects and serve as a preceptor to new associates</li><li>Requires ability to move throughout the hospital for the majority of the workday, with some time spent in office setting</li><li>Must maintain patient dignity and demonstrate cultural sensitivity in care delivery</li></ul>
