Contract Length: 8 weeks (53 days ≈ 8 weeks)

Job Summary

Provide health care services as a Licensed Practical Nurse (LPN) in a home health setting, following prescribed medical treatment and agency policies. Work under the supervision of a Registered Nurse to deliver patient-centered care according to the patient’s plan of care.

Job Responsibilities

• Deliver high-quality nursing care in patients' homes.
• Build meaningful relationships with patients and their families.
• Provide nursing services compliant with agency policies, state and federal regulations, and the patient’s plan of care under RN supervision.
• Observe patients and report any adverse reactions to medication or treatment.
• Assist Physicians and Registered Nurses with the plan of care and specialized procedures.
• Educate patients and families on diet, medication, and treatment.

Required Qualifications

• Valid LPN/LVN license in Florida.
• Minimum of 1 year of hospice experience within the last year.
• Current BLS certification.

Preferred Qualifications

• Intravenous (IV) Certification.

Schedule

• Monday to Friday, 40 hours per week plus weekend rotation.
• Full-time position.

• Not required.
