Registered Nurse: Case Manager

Location: Jeffersonville, Indiana
Employment Type: Temporary Contract
Contract Length: 13 weeks
Work Schedule: Monday to Friday, 8:00 AM – 5:00 PM
On-Call: Yes, weekend on-call rotation required
Hours per Week: 40 hours (not guaranteed)

Job Description

• Perform comprehensive assessments of high-risk members in a home health setting.
• Collaborate with primary care providers to develop and implement individualized, comprehensive care plans with targeted interventions.
• Continuously reassess and address cost-efficient, appropriate levels of care.
• Coordinate non-medical support services (e.g., housing, transportation) to ensure treatment plan compliance.
• Manage care transitions by working with physicians, social workers, discharge planners, and claims professionals.
• Engage specialty resources as needed to optimize patient outcomes.
• Maintain detailed records of clinical, functional, and fiscal outcomes throughout care management.
• Identify opportunities for health promotion and illness prevention.
• Act to prevent adverse patient events and intervene promptly when prevention is not possible.
• Advocate for patients by protecting privacy and confidentiality.

Required Qualifications

• Valid Registered Nurse (RN) license in Indiana
• Minimum of 2 years of case management experience, including experience within the last year
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience in a home health setting
• Skills and experience in IV therapy
• Wound care experience

Additional Information

• Mileage reimbursement provided at $0.46 per mile.
• Primary coverage areas include Bullitt and South Jefferson counties, with flexibility to cover other areas as needed for PTO, call-ins, and admissions.
• Experience with Home Care Home Base (HCHB) software preferred.
• Productivity expectation: 30 points weekly.

Work Environment

• Onsite position with direct patient and provider interaction.
