Pharmacist Position - Houston, Texas

Job Details

Specialty: Pharmacist (Oncology Inpatient focus)
Location: Houston, Texas
Shift: Days
Hours: 10 hours per day, 4 days per week (40 hours total)
Contract Length: 10 weeks
Schedule: 4x10s with on-call rotation

Job Description

Participate in rounding with the patient care team.
Provide recommendations on medication interventions.
Create treatment protocols in Epic/Epic Beacon.
Facilitate transition of care to outpatient oncology pharmacists.
Submit non-formulary requests.
Coordinate specialty medications for patients.
Work in a high-acuity, quaternary academic medical hospital with a complex patient population including advanced cardiac care, oncology, and multi-organ transplantation.

Required Qualifications

Valid Pharmacist license in Texas
Minimum 3 years of pharmacist experience, including experience within the last year
BLS Certification

Additional Information
