Job Details

• Location: Beachwood, Ohio
• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 13 weeks
• Shift: 7:00 AM – 3:30 PM (rotating shifts: 7-3:30 / 9-5:30)
• Schedule: 5 shifts per week, 8 hours each
• Call: Required

Required Qualifications

• Certified Surgical Technologist with a minimum of 1 year of experience, including experience within the last year
• BLS certification issued by the American Heart Association
• Fit Test completed within 1 year prior to start and updated annually

Health and Safety Requirements

• Immunizations and Titers:

  • MMR: Positive titers within last 15 years; booster protocol allowed if low titer.
  • Varicella: Positive titers within last 15 years; booster protocol allowed if low titer.
  • Hepatitis B: Proof of full series or partial series with signed declination; positive titers accepted within 15 years.
  • Tdap: Vaccination within last 10 years; no declinations accepted.
• Tuberculosis Testing:

  • 2-Step PPD or IGRA (QuantiFERON/TSPOT) with negative results within last 6 months (annual requirement).
  • If positive, clear chest X-ray (valid for 5 years) and current TB questionnaire required.
• Fit Test: Completed within 1 year prior to start; updated annually. Specific approved respirators listed.
• Physical Exam: Pre-employment physical exam within 1 year of start date confirming ability to work without restrictions.
• Drug Screen: 10-panel drug screen within 60 days prior to start; instant drug screens not accepted. New screen required if break in assignment >60 days.
• COVID-19 Vaccine: Preferred but not required; medical or religious exemptions accepted with approval.
• Influenza Vaccine: Proof required during flu season (October–March) or mask-wearing and completion of specific declination forms.

Background and Verification Requirements

• Criminal Background Check: Within 6 months of start, including Social Security trace, county searches (last 7 years), national criminal sweep, and sex offender registry. International background check required if not U.S. resident for past 5 years.
• Fingerprints: Ohio residents 5+ years require Ohio BCI&I and national FCRA-compliant background checks; residents less than 5 years require Ohio BCI&I and FBI fingerprint checks. Attestation allowed if fingerprints pending.
• Employment Verification: Verification of employment within last 5 years through third-party; attestations not allowed.
• Education Verification: Highest level of education verified through third-party; international education requires equivalency documentation.
• I-9 Attestation: Proof of eligibility to work in the U.S. required.
• OIG, SAM/EPLS/GSA/OFAC Checks: Must be completed within 6 months prior to start; only required upon initial hire.
• Social Security Trace: Recommended if not included in background check, to be completed within 1 year of start.

Skills and Competency

• Skills Checklist: Clinical skills checklist completed within 1 year prior to start; required once upon initial hire.
• Competency Exam: Must score at least 80% on relevant department exam within 1 year prior to start; required once upon initial hire.
• Core Competencies: Must pass with 80% score; includes HIPAA, Joint Commission NPSG, OSHA topics; completed within 1 year prior to start; required once upon initial hire.
• Safety and Compliance Training: Completion of required safety modules prior to clearance; renewal annually.
