Physical Therapist (PT) Position - Muskegon, Michigan

Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks
Work Schedule:

• Monday to Friday
• Holiday rotation (no weekends or holidays)
• EPIC electronic health record system used

Personal Health History

• COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption with agency letterhead (declination not accepted for CA or IL). Valid through October 2026.
• MMR: Proof of 2 vaccinations with dates or IgG titer lab report.
• Fit Test: Required for Iowa - 1860 mask only.
• Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Hep B titers not accepted.
• Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead. Flu season dates apply (10/1/2024 - 4/30/2025 and 10/1/2025 - 4/30/2026). Declination not accepted for CA or IL.
• Driver's License/State ID: Valid photo ID with all four corners visible plus insurance card photo.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Positive TB requires chest x-ray within last year for Michigan and certain states; otherwise within 5 years.
• Varicella: Proof of 1 vaccination with date or IgG titer lab report.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days), must include specific substances; no THC allowed.
• Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.

Professional Requirements

• Trinity Profile: Resume, skills checklist within a year, and 2 professional references required before submission.
• Confidentiality & Security Agreement: Signed Trinity Health confidentiality and system security agreement required.
• Conflict of Interest Disclosure Form: Signed form required.
• Criminal Background Check: Federal, nationwide, county criminal check from last 7 years plus SSN verification, dated within 30 days of start. Includes employment verifications.
• Education Verification: Third-party screening report verifying highest academic education (degree or diploma/GED). Certifications required if specified.
• Excluded Parties List System (EPLS): Scanned report dated within past 30 days, all aliases searched.
• Fingerprinting: State and department specific.
• GSA/OIG Exclusion Check: Scanned report dated within past 30 days, all aliases searched.
• Integrity and Audit Services Form: Signed within days prior to start.
• Joint Commission Testing: CORE exam within the last year.
• National Sex Offender Registry (NSO): Report dated within 30 days, all aliases searched. Michigan requires statewide check as well.
• System Access Form: Completed and uploaded one week before start.
• Skills Checklist: Provided by agency for clinical contractors.
• BLS Certification: American Heart Association certification required for all direct patient care personnel. Must include front and back images of card with expiration date.
• License/Certificate Verification: Online verification dated within 30 days of start. For California RNs, a picture of the hard copy license is also required.
• Code of Conduct Form: Signed form required.
• I9 Form: Standard I9 completed by agency.
• Specialty Specific Competency Testing: Specialty exam completed within the last year.
• Color Vision: Required (no additional comments provided).

This position requires thorough compliance with health, professional, and legal documentation prior to onboarding. The role involves weekday work with no weekend or holiday shifts, and use of EPIC system.
