Physical Therapist - Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (approximately 13 weeks)
Shift: 8:00 AM – 4:30 PM

Job Summary

Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient ages range from birth to over 100 years. Collaborate with patients, families, physicians, and other health professionals to assess needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice.

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Minimum 1 year of experience as a Physical Therapist, including experience within the last year
• Minimum 1 year of certified home health care experience
• Minimum 1 year of case management experience
• Ability to travel to patient cases as assigned
• Weekend rotations occur approximately once every 6 weeks

Onboarding and Compliance Requirements

• Submission of a recent (within 6 months) professional color photo meeting specific guidelines (plain background, no filters, no eyeglasses).
• Motor Vehicle Record (MVR) check within 45 days of start.
• Primary source verification of professional license/certification within 30 days of start and updated upon expiration.
• N95 respirator fit test within 365 days of start, updated annually (no exemptions allowed).
• Nurse Aide Registry check within 30 days of start (required for all employees).
• Respirator Medical Clearance attestation if fit test required.
• Documentation of MMR vaccination or immunity proof.
• TB test documentation within 3 months of start, updated if there is a break >6 months.
• Physician health statement within 365 days of start, updated if break >6 months.
• Criminal background check within 45 days prior to start, including county and international residence/employment history for the last 7 years.
• Massachusetts CORI background check within 30 days prior to start.
• Submission of a detailed resume covering the past 7 years, including city and state for all work and education.
• Two professional references within the last 4 years, including at least one supervisor or higher.
• Completion of a skills checklist within 1 year of submission.
• Confirmation of assignment form required for all initial assignments and extensions.
• Education verification through background check company.
• Social Security Number trace as part of criminal background check.
• National sex offender registry check within 30 days of start, updated annually.
• Exclusion checks against OIG, GSA, and state Medicaid exclusion lists within 30 days of start and annually.

Additional documentation may be required if legal name changes are present or if positive TB test results require chest x-ray confirmation.
