Required Qualifications

• Valid RN license in Indiana
• Stepdown (2 years of experience)
• Stepdown experience within the last year
• BLS issued by American Heart Association
• ACLS issued by American Heart Association
• NIHSS
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License
• Stroke experience

Preferred

• Fit Test — required for all travelers; preferred to be completed prior to start, but can be completed onsite

Additional Notes

• Influenza vaccine exemptions/declinations are accepted, but a mask must be worn at all times.
• COVID-19 vaccination requires full vaccination; declinations are not accepted.
• MMR requires documented proof of two vaccines or positive IGG titer; declinations are not accepted.
• Tdap declinations or attestations are not accepted.
• Varicella requires documented proof of two vaccines, serologic immunity, or provider verification of history; declinations are not accepted.
• Health statement must be a statement of good health or completed physical exam prior to first assignment, updated annually.
• Driver's license/state ID must not be expired and must match the address on file.
• If positive TB history is present, chest X-ray proof within the last year is required with the TB questionnaire.
• Ascension Skills Checklist is clinical only and must be completed within one year of start, then updated annually for specialty.
• Ascension Pharmacology Exam, Medical Surgical Exam, Core Competencies, and EKG Exam must be completed before start and are updated on a recurring schedule.
• Ascension 12 Panel Drug Screen must be completed within 30 days prior to the start of the first assignment and updated annually.
• Education verification must be relevant to the candidate’s clinical work and completed within 90 days of start.
