• FMOLHS Criminal Background Check (Required for onboarding; must be completed within 30 days prior to start, including multi-county and national checks, education and employment verification, driving record)
• FMOLHS Badge Photo (Required for onboarding; forward-facing, well-lit JPEG)
• FMOLHS Contract Annual Education Packet (Required for onboarding)

Personal Health History Requirements (All required for onboarding unless noted)

• COVID-19 Vaccine (Optional; proof or exemption accepted)
• MMR (Proof of 2 immunizations, positive titer, or declination with signed statement)
• Fit Test (N95 mask fit test within 1 year, updated annually)
• Hepatitis B (Proof of 3 immunizations, positive titer, or declination with required signatures)
• Influenza Vaccine (Required during flu season; medical/religious exemptions accepted with documentation)
• TB Test (Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, MD note or treatment proof required)
• Health Statement (Signed within 1 year stating ability to work without restrictions)
• Varicella (Proof of 2 immunizations, positive titer, or declination with signed statement)
• Tdap (Vaccination within past 10 years or declination with signatures)
• Annual TB Questionnaire (Required within 6 months of start; evaluation and treatment proof if positive)
• FMOLHS Drug Screen (Negative 10-panel drug screen within 30 days prior to start)
• FMOLHS Eye Exam & Color Vision (Ishihara Color Test and Snellen Vision test within 1 year)

Checklist/Testing Requirements (Required for onboarding)

• FMOLHS Skills Checklist (Unit-specific, within 1 year, updated annually)
• FMOLHS Competency Exam (Passing score of 80% or higher within 1 year)
• FMOLHS Core Competencies (Passing score of 80% or higher within 1 year; includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; updated annually)

This position requires compliance with all listed certifications, licenses, health screenings, and competency testing prior to onboarding.
