Assignment Details

• Position: Full-time physical therapy (PT) in home health
• Contract Length: 13 weeks
• Schedule: Day shift, 5 days a week, 8 hours a day (8:00 to 5:00), 40 hours per week

Required Qualifications

• Valid Physical Therapist license in Ohio
• Basic Life Support (BLS) certification

Health and Immunization Requirements

• TB screening: Negative 2-step PPD, QuantiFERON-Gold, or T-SPOT within 1 year before start. If previously positive, a negative chest X-ray from the last 5 years is required.
• Measles, mumps, rubella, and varicella: Two vaccines or a positive titer. For negative or equivocal titers, a declination, two vaccines, or one booster is required. Varicella may also be met with a history of chickenpox. Candidates born before 1957 are exempt.
• Hepatitis B: Facility-specific form required. If accepting the vaccine, proof of vaccination is required. If declining, no other documentation is needed.
• Physical: Physical statement dated within 12 months before start is required.
• Flu: Vaccine or approved exemption required during flu season (November to April) for home health and hospice candidates at certain locations.
• Drug testing: 9-panel drug screen required for all candidates.

Background Checks and Screening

• Background check completed at the time of offer (between assignment acceptance and start date) for Ohio.
• Searches include SSN trace, MVR, FACIS Level 3, nationwide sex offender registry, state, nationwide, and federal criminal databases, and county records.
• Ohio BCI/FBI fingerprinting completed at the time of offer (between assignment acceptance and start date). Results must be provided once received if fingerprints are taken after the start date.
