Job Description

• Position in outpatient orthopedic physical therapy at a city location.

Experience Requirements

• Minimum of 1 year experience in the specialty or one of comparable or higher acuity is required.
• No explicit mention of prior travel experience required.

Certifications & Licenses

• Basic Life Support (BLS) - Required for all candidates (AHA provider or AHA RQI accepted; RQI provider like Laerdal not accepted).
• Advanced Cardiac Life Support (ACLS) - Required for specific RN/Unit positions, not required for this specialty.
• State License verification required within 30 days of assignment start.
• Illinois Department of Public Health (IDPH) fingerprinting required if the profession does not require an Illinois state license.

Background & Verification Requirements

• 7-year background check including aliases, sex offender, counties, national federal, criminal, FACIS 3, OFAC.
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start.
• Education verification required through a 3rd party vendor for highest level of education; diploma/transcripts acceptable if verification unavailable.
• Employment verification for the last 7 years required.
• Skills checklist for assigned and float units, updated annually.
• References: 2 required.
• Current resume including last 7 years of employment with explanations for gaps over 30 days.

Health & Immunization Requirements

• Physical exam within 1 year, updated annually, confirming good health.
• Fit mask test within 1 year of start (specific mask models required).
• Negative 9-panel drug test within 30 days of start.
• Immunizations or proof of immunity (or declination where allowed) required within 2 business days of contract signing for:

  • Tetanus/Diphtheria/Tdap
  • TB test or chest X-ray (negative PPD, Qgold, or Tspot within 1 year)
  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• TB questionnaire required if positive TB test.
• Seasonal flu vaccination/documentation required for the 2025-26 season; religious or medical exemptions require facility approval. Declinations not accepted.

Facility Specific Requirements

• Photo for badge (passport style, plain background, forward facing, no headgear).
• Completion of facility orientation modules (~2 hours) prior to clearance/start.
• COVID-19 and Influenza religious and medical exemption forms available and require timely submission and approval if applicable.

Additional Notes

• Fingerprinting must be completed in Illinois and added to the Illinois Department of Public Health Registry if no state license is required.
• Candidates have 10 days from signed authorization to complete fingerprinting.
• Background checks and verifications must be current and completed timely to avoid delays.
