Job Opportunity: Physical Therapist (PT) - Home Health

Location: San Jose, California
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Work schedule: Monday to Friday, 8 AM to 5 PM (5x8's)
  • Area of coverage: San Jose/South Bay/Santa Clara County
  • Must have California PT license and BLS certification in hand to submit
  • Home Health experience required

Required Qualifications

  • License: California Physical Therapist (PT) License
  • Certifications:
    • BLS (Basic Life Support)
  • Experience: Minimum 2 years of experience in Physical Therapy
  • Special Requirements: Prior Home Health experience

Background and Compliance Requirements

  • 7-year background check required within 6 months of start
  • Motor Vehicle Report required (due to driving position)
  • Education verification and transcript/diploma verification as applicable
  • FACIS Level 3 screening required within 2 weeks of start and monthly thereafter
  • Sex offender search required within 90 days of start
  • Various federal and state background checks (OIG, EPLS/GSA/SAM, OFAC, Medi-Cal Suspended and Ineligible Provider List) required and updated annually
  • TB Screening Questionnaire required within 90 days of start and annually thereafter
  • Physical exam required within 1 year of start
  • Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law)
  • Immunizations and titers required:
    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Measles (Rubeola), Mumps, Rubella (MMR)
    • Hepatitis B series or declination
    • Varicella
    • Seasonal Flu vaccination during flu season (Nov-April) or signed declination
  • Fit mask test required within 1 year of start and annually (with accommodations for religious exemptions or failed tests)
  • Respiratory safety clearance required prior to start and annually (not required for some non-clinical roles)

Facility Specific Documents and Training

  • Candidates must review and sign facility-specific documents including:
    • Job Description
    • Standards of Business Conduct
    • Code of Conduct
    • Vendor Training (2024 and 2025 versions depending on contract date)
    • Overpayment Refund Policy
    • General Regulatory Orientation for Contracted Workers
  • Religious accommodation forms required if applicable (for facial hair PAPR exemption or vaccine exemptions)

Additional Requirements

  • Employment application must include full 7 years of work history with no gaps
  • Verification of employment history up to 7 years required
  • References: 2 professional references required (Aya requirement, not client)
  • Unit-specific skills checklist required and must be renewed annually (Aya requirement)
  • Competency exams and medication/pharmacology tests may be required as applicable
