Job Details

  • Position: Acute Care Physical Therapist
  • 1 Year contract, first-timers welcome
  • No floating between units
  • No on-call required
  • No weekend shifts required
  • No block schedules required

  • Total Staffed Beds: 379
  • Level II Trauma Center

Uniform/Dress Code

  • Licensed nursing personnel (RN, LVN): solid navy blue uniforms or scrubs (no print tops except in Children’s Hospital and Women’s Services units)
  • Unlicensed personnel (US, CNA, PCA, NT, MT): solid hunter green uniforms or scrubs (no print tops except noted exceptions)

Parking

  • Free parking available in unrestricted spaces and designated employee/visitor surface lots

Required Certifications (as applicable)

  • ACLS (Advanced Cardiac Life Support) – AHA or Canadian Heart & Stroke Foundation certification
  • BLS (Basic Life Support) – AHA or Canadian Heart & Stroke Foundation certification
  • PALS (Pediatric Advanced Life Support) – AHA or Canadian Heart & Stroke Foundation certification
  • NRP (Neonatal Resuscitation Program) – Current certification by AAP, AHA, or Canadian Paediatric Society
  • Chemotherapy certification (if applicable)
  • Electronic Fetal Monitoring (EFM) certification (if applicable)
  • State professional license(s) as required
  • IV Certification (for LVN/LPN/Other as applicable)
  • Management of Assaultive Behavior (MAB) (if required)
  • NIH Stroke Certification (renew every 2 years if required)
  • CPI (Crisis Prevention Institute) certification (if required)

Background and Compliance Requirements

  • Background check within 45 days of start, covering all locations lived/worked in past 7 years, updated annually
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
  • Education Verification required for all candidates
  • State-specific searches as applicable (e.g., Washington WSP/WATCH, OBRA Registry for CNAs)
  • Drug testing: 10-panel drug screen within 90 days of start (no instant tests)
  • References: Minimum 2 supervisor references covering 1 year employment in last 3 years (6 months for CNAs)
  • Employment application covering at least last 5 years (7 years for APN/Subcontractors)
  • Skills checklists and competency exams as applicable

Health and Safety Requirements

  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit mask test (N95 or equivalent) required for travel candidates, updated annually
  • Immunizations/vaccinations required or declination forms accepted:
    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Negative TB test or chest X-ray within 12 months, updated annually
    • Measles (Rubeola), Mumps, Rubella (2 MMR vaccines or positive titer)
    • Hepatitis B vaccination or signed declination
    • Varicella (2 vaccines or positive titer)
    • Seasonal Flu vaccine or declination (required during flu season)
    • COVID-19 vaccine or declination
  • Respiratory Safety Clearance for respirator use, medically cleared annually

Competency and Training Requirements

  • Medication Test/Pharmacology Competency Exam (as applicable)
  • Color Blind Testing (if required by unit)
  • Competency exams relevant to specialty
  • Specialty-specific competency exams (e.g., Medication Aide exam for CNAs in LTC Oregon facilities)
  • WorkRamp LMS modules:
    • Core modules for all caregivers (except some non-clinical roles)
    • Specialty and regional modules as applicable
    • Epic training modules (for certain regions and roles)

Additional Notes

  • Float Policy Acknowledgement required (may be required to float to other Providence facilities nearby)
  • Passport-style badge photo required
  • Some facility-specific documents and attestations may be required depending on placement
  • Modules and training must be completed prior to or shortly after start date as assigned
