Required Credentials & Documentation

Identification

• Current and valid Driver's License, government photo ID, or passport
• Proof of name change if applicable (certifications/licenses must have current legal name)

Background Checks (required within 30 days of start)

• 7-year background check (renew annually) including:

  • SS Trace
  • Nationwide, Counties, Statewide, National Criminal, National Sex Offender
  • OFAC, FACIS L1
• EPLS/GSA/SAM
• HHS/OIG
• Education verification (3rd party verification of highest education level)

Certifications (AHA only where specified; certificates of completion NOT accepted)

• ACLS (Advanced Cardiac Life Support) – required for ER, PCU, CVICU, ICU, Cath Lab, LAD, IR, Urgent Care
• BLS (Basic Life Support)
• PALS (Pediatric Advanced Life Support) – required for ER, NICU, PICU, Pediatrics, Interventional Radiology, Urgent Care
• NRP (Neonatal Resuscitation Procedure) – required for Nursery, NICU, L&D, Mother/Baby
• TNCC (Trauma Nursing Core Course) – required for ER
• EFM (Electronic Fetal Monitoring) – AWHONN Intermediate required for L&D; AWHONN Advanced also accepted
• Technical Certification Verification (national certifications required for Radiology Techs, Surgical Techs, Phlebotomists, etc.)
• State License Verification (MT state license required for Radiology Techs; temp licenses NOT accepted)
• NURSYS Verification for RNs and LPN/LVNs (required within 30 days of start)
• CPI (Crisis Prevention Institute) – preferred for Psych

Competencies

• Dysrhythmia/EKG Competency Exam – preferred for Tele, Stepdown, PCU, ICU, ER (80% pass rate)

Health & Safety Requirements

Physical & Immunizations

• Physical exam within 1 year of start, signed by MD, NP, or PA (must state candidate is in good health, free from communicable diseases, and able to perform job duties)
• Fit mask quantitative fit test with 3M mask within 90 days prior to start (qualitative tests not accepted; must be clean-shaven; annual renewal required)
• Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination (TD not accepted)
• Negative TB test or chest X-ray (annual renewal required) with options for testing protocols
• TB questionnaire for past positive TB cases (annual renewal)
• Required immunizations or positive IGG titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (positive IGG titer, 3-dose vaccine series, or declination within last year)
  • Varicella
• Seasonal flu vaccination/documentation required October 31 - March 31 (declinations and exemptions accepted with proper documentation)
• COVID vaccination initial series or approved medical/religious exemption required

Drug Testing

• Negative drug screen within 30 days of start, renewed annually
• Must include testing for amphetamines, MDMA, barbiturates, benzodiazepines, cannabinoids, cocaine, opiates, fentanyl, methadone, tramadol, buprenorphine, and others

Employment History & References

• 2 references required, including a direct supervisor reference within the last year (letters accepted)
• Current employment application covering 7 years with explanations for gaps over 30 days
• Employment verification preferred for last 5 years
• Skills checklist required as applicable

Facility Specific Requirements

• OSHA form required for onsite fit testing (must be signed and submitted with lab authorization)
