Job Description

• Specialty: Home Health Physical Therapy

• Territory: Up to a 60-mile radius from the hub/office

• Special Requirements:

  • Flexibility with territory expected for travelers
  • Timely documentation and communication required
• Dress Code: Scrubs (any color), no jeans
• Technology Provided: iPad provided (cell phone not allowed)
• Mileage: Reimbursed at IRS standard rate

Required Certifications & Compliance

• BLS (Basic Life Support): Required for clinical positions (AHA, ARC, Military accepted)
• Orientation Safety Manual Acknowledgement: Signed annually
• State License Verification: Primary source verification within 90 days of start and upon expiration
• NURSYS Verification: For RN, LPN/LVN, AP as applicable within 90 days of start
• Medication/Pharmacology Competency Exam: As applicable by profession
• Competency Exam: As applicable to profession/specialty
• Background Check: Required within 90 days of start and updated annually, including:

  • SSN Trace
  • County searches for last 7 years
  • FACIS Level 1
  • Nationwide Sex Offender
  • Motor Vehicle Records (MVR)
  • Education and Work History Verification
  • State-specific checks as applicable
• EPLS/GSA/SAM Checks: Required within 90 days of start
• References:

  • 2 professional references for travel, allied, per diem acute care
  • 1 professional reference for per diem non-acute care
  • References must be within 2 years of start
• Employment Application: Full 7 years of work history with no gaps
• Skills Checklist: Required annually
• Fingerprinting: Required for certain states (e.g., Connecticut, Ohio) with specific instructions and timelines

Health & Immunization Requirements

• Physical Exam: Physician statement within last 12 months
• TB Testing: Negative TB test or chest X-ray within required timeframes
• TB Questionnaire: Required annually
• Immunizations:

  • Rubeola (Measles): 2 vaccines or positive titer (exempt if born before 1957)
  • Mumps: 2 vaccines or positive titer (exempt if born before 1957)
  • Rubella: 2 vaccines or positive titer (exempt if born before 1957)
  • Hepatitis B: Series, titer, or declination accepted
  • Varicella: Series, titer, or declination accepted
• Drug Testing: 9-panel drug screen within last 12 months
