Job Details

• Specialty: Home Health Registered Nurse (RN)

Required Background Checks & Verifications

• Background check within 90 days of start, updated annually, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide Sex Offender
  • Motor Vehicle Records (MVR)
  • Education Verification
  • Work History Verification
• State-specific background checks as applicable (e.g., Kentucky, Virginia)
• EPLS/GSA/SAM check within 90 days of start
• HHS/OIG check within 90 days of start
• Fingerprinting as required by state (e.g., Connecticut, Ohio)
• Education verification of highest degree related to profession
• State-specific registry checks (e.g., Nurse Aide Abuse Registry)

Certifications & Competencies

• BLS (Basic Life Support) certification required (AHA, ARC, Military accepted)
• Orientation Safety Manual Acknowledgement (annual)
• State License verification (primary source, within 90 days of start and upon expiration)
• NURSYS verification (for RN, LPN/LVN, AP) within 90 days of start
• Medication Test/Pharmacology Competency Exam as applicable
• Competency Exam as applicable to specialty

Facility-Specific & Compliance Documents

• Proof of current auto insurance (required for all Home Health positions involving driving)
• Acceptable Use Policy (must be completed prior to start)
• Agency Background Attestation (must be signed and on file)
• Flu and COVID Statement (required if declining flu vaccine)
• State-specific forms as applicable (e.g., Missouri Crime Disclosure)

Health Requirements

• Physical exam within last 12 months (not required annually)
• Negative TB test or chest X-ray (PPD, Q Gold, or TSpot within 1 year; chest X-ray within 5 years if positive PPD)
• TB Questionnaire (annual renewal required)
• Immunizations or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9 Panel Drug Screen within last 12 months (instant acceptable)
• Flu seasonal vaccination or facility-specific declination form during flu season (Aug-Nov)
• COVID-19 vaccination or medical/religious exemption (including booster)

Employment History & References

• Full 7 years of work history with no gaps required on application
• Work history verification for 7 years (must be run within 90 days of start)
• 2 professional references required for travel, allied, per diem acute care roles
• 1 professional reference required for per diem non-acute care roles
• References must be recent (within 2 years) and for some states completed within 90 days of start

• Travelers must be flexible with territory and documentation requirements
• Timely communication and compliance with all credentialing and onboarding steps is essential
• Scrubs dress code only; no jeans allowed on assignment
