Location: Hazard, Kentucky
Contract Length: 13 weeks

Work schedule: Four 10-hour day shifts (7:00 AM - 5:30 PM), Monday through Friday
On-call requirements: 1-2 calls during the week and 1-2 weekends per month
Float within scope of practice is required
Must have housing within 30 minutes for STEMI (heart attack) call response
Nurse-to-patient ratios may fluctuate based on census and call-ins
Orientation occurs bi-weekly
Candidate must reside within 75 miles of the facility to be considered

Valid RN license in Kentucky
Minimum 2 years of Cardiac Cath Lab experience, with at least some experience within the last year
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
American Heart Association Basic Life Support (BLS) certification
American Heart Association Advanced Cardiac Life Support (ACLS) certification
NIH Stroke Scale (NIHSS) certification

Annual physical exam or physician statement confirming ability to work without restrictions
Drug test within 30 days of start using a specific 9-panel bundle (no THC) with expanded opiates and oxycodone
Immunizations and proof required prior to start:
- Tetanus/Diphtheria/Tdap (Td not accepted; declinations accepted if within 1 year)
- Measles, Mumps, Rubella (2 MMR vaccinations or positive titer)
- Varicella (2 vaccinations or positive titer)
- Hepatitis B series, titer, or declination (declination or booster accepted if titers negative or equivocal)
Negative TB test via QuantiFERON blood test within 1 year (T-Spot/PPD not accepted); if positive, chest X-ray within 5 years plus TB questionnaire required
Seasonal flu vaccination or approved medical/religious exemption required during flu season
Fit mask test completed onsite (PAPRs not accommodated; exemptions reviewed onsite)

Unexpired government-issued photo ID required
Background checks including national criminal search, sex offender registry, fugitive database, county searches for last 7 years, and federal exclusions lists must be completed within 30 days prior to start
Education verification included in background or third-party verification
State license primary source verification prior to start and upon expiration
NURSYS verification required prior to start
Annual renewal of competencies and certifications as applicable
Completion of facility-specific job description acknowledgment and safety manual
Employee health attestation including TB documentation, OSHA respirator medical evaluation, and flu vaccine/exemption upload required before start
Kentucky Vulnerable Adult Maltreatment Registry verification within 30 days prior to start (annual update required)
West Virginia APS/CPS search results required (annual update required)
