Location: Chicago, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Summary

Cardiovascular/Thoracic Operating Room Registered Nurse needed
Must rank 4/4 in cardiovascular/thoracic OR skills
Day shift: 7:00 AM – 3:30 PM (5 days a week, 8 hours per day, 40 hours per week)
Local nurses welcome at the same rate

Required Qualifications

Valid Registered Nurse (RN) license in Illinois
Minimum of 1 year of Cardiovascular Operating Room (CVOR) experience within the last year
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Operating Room (OR) experience

Required Documentation

Government-issued photo ID (Driver’s License or State ID)
Employment application with 7 years of employment history and explanations for gaps over 30 days
Two professional references within 3 years (for direct hire only)
Completion of facility-specific forms including contractor data form, TB questionnaire, Hep B declination (if applicable), flu exemption (if applicable), and mandated reporter form
Annual TB questionnaire update required
Orientation safety manual acknowledgement

Additional Information

Flu vaccination required for 2025-2026 season unless medically or religiously exempt with approved documentation
Fit mask testing completed on-site (not required for non-clinical staff)
COVID vaccination documentation not required but recommended to collect if vaccinated
Competency exams and skills checklists must be updated annually as applicable
