Audiologist – Travel/Contract Position
Location: San Leandro, California
Contract Length: 13 weeks
Job Details
- Setting: Outpatient audiology clinic (standalone, satellite location)
- Schedule: Block shift, 8:00 AM – 4:30 PM (Day shift, 5 days/week, 40 hours/week)
- Average Daily Census: 7-8 patients
- Coordination: Must coordinate scheduling with ENT department located at a different site
- Special Equipment Used:
- Audiometer
- Middle ear analyzer
- Auditory brain stem response analyzer (ABR)
- Portable audiometer (air conduction)
Required Qualifications
- Valid Audiologist license in California
- Minimum 2 years of audiologist experience, including experience within the last year
- Basic Life Support (BLS) certification
Required Documentation & Compliance
- Photo ID: Valid Driver’s License or Passport (color photo, clearly visible)
- Background Checks: Must be completed within 90 days prior to start, including:
- SSN Trace
- 7-year Nationwide, County, and National Criminal searches
- FACIS Level 3
- National Sex Offender Registry
- OIG Sanction Search
- EPLS/GSA/SAM verification
- Education Verification: Highest level of education must be verified and meet minimum requirements
- Physical Exam: Within 90 days of start, signed by MD, DO, PA, NP, or APRN confirming good health and ability to perform job duties
- Fit Mask Test: Within 90 days of start; acceptable masks include 3M 8210, 3M 1860S, 3M 1860R; PAPR accepted if fit test failed (approval required)
- Immunizations & Health Requirements:
- Tetanus/Diphtheria/Tdap within 10 years or signed declination (TD not accepted)
- Negative TB test or chest X-ray updated annually (IGRA or 2-step PPD)
- TB Questionnaire updated annually
- Proof of immunity or vaccination for:
- Measles (Rubeola)
- Mumps
- Rubella
- Varicella
- Hepatitis B (positive titer or completed series/declination)
- Seasonal Flu Vaccination or declination required annually by November 6; masking required during flu season (Nov 1 – Mar 31)
- Drug Testing: Oral saliva drug screen within 30 days of start; rapid tests not accepted; must cover specified analytes including amphetamines, cocaine, marijuana, opiates, oxycodone, PCP
- Color Blind Exam: Within 90 days of start (Ishihara test accepted)
- Employment History: Verification of past 3 years of employment required
- Resume: Current resume including last 7 years of employment required
- References: Two references required per agency standards (not facility requirement)
Orientation & Competencies
- Completion of facility-specific competencies and New Employee Orientation modules required on first day of work
- Competencies must be completed by 11:59 PM PST on first day; failure to complete within 14 days results in immediate termination
- Badge photo (passport style, no filters) required for facility ID
Additional Notes
- Charge experience preferred but not required
- No prior travel experience required for this position
