PT Home Health - Travel/Contract

Location: San Ramon, California
Contract Length: 13 weeks

Job Details

• Setting: Home Health
• Shift: Day shift, 8 hours per day (7:00 AM - 4:00 PM), 5 days a week
• Weekly Hours: 40 hours (guaranteed 36 hours weekly, with expectation to see 5-6 patients daily)
• Patient Load: 1:1 ratio; may visit up to 6 patients per day (27 productivity points standard)
• Weekend Work: Optional weekend patient visits to meet productivity goals
• On-Call: Not required
• Transportation: Must have own transportation; occasional travel radius up to 50+ miles
• Documentation: Timely communication and documentation required; documentation tools provided and must be returned
• Attire: Any color scrubs
• Parking: Free parking available
• Mileage Reimbursement: IRS standard

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of Physical Therapist experience, including experience within the last year
• Experience in a Home Health setting
• Current Basic Life Support (BLS) certification

Compliance & Onboarding Requirements

• Government-issued photo ID (Driver’s License/State ID)
• Emergency Contact Form
• I9 Attestation
• Background Checks including 7-year work/living history, EPLS/GSA/SAM, and HHS/OIG (updated annually)
• Physical Exam within 1 year, cleared to work without restrictions
• Immunizations/Vaccinations:

  • Tdap within 10 years or annual declination
  • Negative TB test or chest X-ray within 1 year
  • Measles, Mumps, Rubella: 2 vaccines or positive titers within required timeframes
  • Hepatitis B: Titer within 5 years, vaccine series or declination
  • Varicella: 2 vaccines or positive titer
  • Flu vaccine or declination required year-round
  • COVID-19 vaccine documentation or declination form required
• Drug Screening: 10-panel drug screen within 1 year
• Auto Insurance and Registration (must be current throughout assignment)
• Professional References: 2 for travel/allied/per diem acute care roles (within 3 years)
• Work History Verification: 1 year related employment verification required
• Annual Skills Checklist and Competency Exams as applicable

Additional Notes

• The system groups clients to minimize drive and windshield time
• First two weeks may have reduced guaranteed hours due to ramp-up, holidays, or time off
• Locals are accepted for this travel/contract position

This position is ideal for experienced Home Health Physical Therapists licensed in California who are comfortable managing a full patient load with independent transportation and strong documentation skills.
