Job Opportunity: Social Worker II

Job Description

Under general supervision, provide social services to patients and their families. Key responsibilities include:

  • Providing social services for an assigned medical service
  • Conducting discharge planning evaluations
  • Performing psychosocial assessments and communicating findings to hospital staff
  • Counseling patients and families on diseases, treatments, social/financial situations, emotional stress, and other issues
  • Conducting high social risk case findings and crisis counseling (e.g., child abuse, neglect, physical abuse)
  • Providing crisis intervention services
  • Acting as a liaison between patients/families and hospital staff or other agencies
  • Evaluating effectiveness of services and adjusting patient care as needed
  • Assisting with discharge planning and community referrals
  • Supporting patients and families with post-hospital placement or transfer to post-acute care
  • Helping patients and families adjust to illness, treatment, or changes in functional status
  • Using specialized communication skills with children
  • Advocating for patients on issues like informed decision making, end-of-life care, goal setting, and ethical dilemmas
  • May prepare statistics related to referrals and extended stays
  • May supervise students or newly hired employees

Required Qualifications

  • Master’s Degree in Social Work from an accredited school
  • Licensed Master Social Worker (LMSW) strongly preferred
  • Hospital experience preferred
  • Knowledge of social and psychological casework principles and practices
  • Familiarity with social and welfare resources in the community and hospital
  • Strong analytical skills to interpret situations and determine appropriate actions
  • Ability to explain technical information clearly to patients, families, and staff
  • Good interpersonal skills
