Location: Fayetteville, North Carolina
Employment Type: Staffing - Supplier Sourced
Contract Length: 12 weeks (81 days ≈ 12 weeks)
Job Summary
- Provide direct patient care services to hospice patients and supportive care to their families.
- Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality.
- Case manage all assigned patient clinical records.
- Perform initial and ongoing patient assessments.
- Develop and implement Hospice Plan of Care in collaboration with physicians and interdisciplinary team members.
- Deliver comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
- Report changes in patient condition to attending and hospice physicians.
- Maintain accurate and updated medical records, including interdisciplinary notes and plans of care.
- Practice infection control and standard precautions.
- Monitor staff scheduling and participate in quality assessment and performance improvement programs.
- May participate in agency on-call schedule.
- Maintain knowledge of pain and symptom management in the dying process.
- Conduct supervisory visits with team members at least monthly.
- Maintain close contact with patients’ families to provide information, support, and continuity of care.
- Act as primary liaison between physician, patient/family, and hospice team.
- Educate patients, families, and caregivers on medication administration, dressing changes, oxygen use, hospice philosophy, and terminal care.
- Supervise aides and perform home visits at least every two weeks to assess care and services.
Required Skills & Experience
- Minimum 1 year RN experience in acute care hospital, oncology, long-term care, hospice, or home health (school internships and clinical hours may be considered).
- Knowledge of care for terminally ill patients and their families, with understanding of hospice needs.
- Strong patient assessment skills and knowledge of roles within hospice disciplines.
- Familiarity with community resources, documentation, program management, and regulatory guidelines (Medicare, Medicaid, ACHC).
- Proficient in nursing assessment tools and basic office technology (computers, fax, copier, cell phone).
- Must have and maintain transportation for work.
