LPN/LVN - Rehab (Travel Position)
Location: Madison, Wisconsin
Contract Length: 13 weeks
Job Description
- Provide nursing care in a 49-bed acute inpatient rehabilitation facility specializing in stroke, brain and spinal cord injuries, amputations, complex orthopedic injuries, and other conditions requiring inpatient rehab.
- Patient population consists of adults with high acuity needs, often referred from a Trauma Center.
- The unit functions similarly to a medical-surgical unit with high acuity patients.
- Nurses are expected to assist CNAs and respond to call lights to ensure patient safety.
- Patient ratio is approximately 1 nurse to 7 patients.
- Equipment includes mobility aids (ceiling lifts, Sara Stedy, Sara Plus, slide boards), vital sign machines, EKG, blood glucose monitors, and computers for charting.
- Electronic Health Record system used: EPIC.
Work Schedule & Environment
- Shift: 12-hour shifts, from 6:30 PM to 7:00 AM (night shift).
- Weekend and holiday rotations every other weekend.
- Call requirements apply.
- Mandatory 30-minute unpaid break per shift (two 15-minute paid breaks included).
- Staff must adhere to attendance and tardy policies.
- Floating to a nearby satellite inpatient rehab facility (within 20 minutes) may be required in the future.
Required Qualifications
- Valid LPN / LVN license in Wisconsin
- Minimum 2 years of LPN / LVN nursing experience, including experience within the last year
- Experience in a rehabilitation setting
- Medical-surgical nursing experience
- Experience with EPIC Electronic Health Record system
- Basic Life Support (BLS) certification

- Local candidates will not be accepted for this travel assignment.
- The facility is accredited by The Joint Commission and CARF for comprehensive inpatient rehabilitation and stroke specialty programs.
This position offers an opportunity to work in a specialized inpatient rehab setting with a focus on high-acuity adult patients requiring comprehensive rehabilitative care.
