Job Description

• Support patients and family members through the final stages of illness in a hospice setting.
• Services provided in patients' homes, skilled nursing facilities, assisted living facilities, and adult foster homes.
• Responsibilities include intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, and bereavement support.
• Medications and equipment are provided as appropriate.

Qualifications & Requirements

• Bachelor's degree in social services, counseling, or psychology required; Master's degree preferred.
• Healthcare Provider Level BLS certification required.
• Experience or education in grief or loss counseling required.
• Recent experience in hospice preferred.
• Additional experience in a medical setting preferred.
• Use of personal automobile required with valid driver's license and auto insurance. Vehicle must be maintained in good working order.
