As an RN Case Manager in Home Health/Hospice, you will:

Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
Coordinate care plans through interdisciplinary communication and case conferences.
Provide ongoing assessment of patients’ clinical, functional, and emotional status, communicating significant changes to physicians and team members.
Document evaluations, treatment goals, and objectives.
Teach patients and caregivers self-care techniques, medication, diet, and provide health counseling as appropriate.

This role offers broad independence while being part of a cohesive patient-care team, with a focus on work/life balance and flexibility.

Licensed Registered Nurse (RN) in Vermont and New Hampshire or hold an unencumbered compact license.
Graduate of an accredited school of nursing.
Minimum 2 years of experience in Home Health and Hospice nursing.
