Job Description

• Work in a hospital-based skilled nursing facility specializing in geriatric nursing.
• Provide supervised restorative care for older patients recovering from hospitalization or acute illness.
• Focus on improving patients’ health skills and functional ability within the community.
• Collaborate with medicine, nursing, respiratory therapy, and other departments to support patient restoration.

Unit Details

• Unit: UTCC/Transitional Care Unit (TCU)
• Beds: 32
• Nurse-to-patient ratio: 1:9-12
• Nurse Aides ratio: 1:12
• Charge Nurse handles admissions and physician orders
• Systems used: EPIC
• Orientation: 1-2 shifts
• Shifts: 2 (7 AM and 7 PM)
• Weekend schedule: Rotate every other weekend
• Holidays: Rotate
• Floating: No
• Scrub color: Royal blue
• Telemetry: No
• Ventilators: No
• IV Team available, but nurse should attempt IV starts first
• Respiratory Therapy available 24/7
• Pharmacy available 24/7

Patient Population

• Post-operative patients with hips, knees, shoulders surgeries
• Patients with diagnoses such as pneumonia and congestive heart failure (CHF) requiring extra rehabilitation before discharge

Required Qualifications

• Valid RN license in West Virginia
• Minimum 2 years of experience in a Transitional Care Unit, including experience within the last year
• Experience in Long Term Care and Skilled Nursing
• BLS Certification
• WV Cares certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience working with geriatric patients
