Job Opportunity: Physical Therapy Assistant (PTA) - Home Health

Location: Sacramento, California
Contract Length: 13 weeks

Job Description

Seeking an experienced Home Health Physical Therapy Assistant (PTA) traveler to provide physical therapy services in a home health setting. This role involves assessing, evaluating, and updating patients' therapeutic and functional status, developing and implementing plans of care, and instructing patients and caregivers. The PTA will also supervise home health aides, provide consultations, and participate in staff competency assessments.

Required Qualifications

• Minimum 2 years of experience in healthcare, with Home Health experience including OASIS strongly preferred
• Active California Physical Therapy Assistant (PTA) license
• Current BLS certification
• Active NPI number
• Valid driver’s license and proof of automobile insurance
• Good oral and written communication skills and strong organizational skills
• EMR experience with HomeCare Home Base (HCHB) preferred
• Candidates must not have been employed (full-time, part-time, or PRN) by any facility associated with CommonSpirit (including CHI or Dignity) within the past year; PRN employment through outside firms is acceptable

Key Responsibilities

• Provide physical therapy services according to physician’s plan of care
• Assess home environment and patient’s muscle strength, mobility, gait, and range of motion
• Instruct patients, families, and caregivers on therapeutic appliance use and physical therapy goals
• Report patient progress and changes in condition to physicians
• Initiate physical therapy programs and instruct other personnel or family members involved in patient care
• Supervise homemaker/Home Health Aides participating in rehabilitation
• Identify and refer patient needs for other home health services
• Prepare and submit clinical and progress summaries
• Provide physical therapy consultations and inservice education as needed
• Participate in peer consultation and ongoing staff competency assessments
• Comply with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other relevant regulations

Additional Information

• Position is for LOA coverage starting June 24
• Work schedule: 8-hour days, every other week (EOW)
• Mileage reimbursement available per GSA guidelines
