Job Opportunity: Pharmacist Outpatient (Travel/Contract)

Location: San Luis Obispo, California
Contract Length: 13 weeks

Position Details

• Floating: May float to sister facilities as needed

Required Experience & Qualifications

• Minimum 2 years of experience in pharmacy
• Sterile Compounding IV room experience required (both hazardous and non-hazardous drugs)
• PharmD degree required
• California Pharmacist license required
• First-time travelers are eligible if experience requirements are met
• No weekend shifts required
• No on-call required

Required Documentation & Compliance

• Current and valid Driver’s License or government-issued photo ID
• Background checks within 30 days of start date, including:

  • Social Security Trace
  • 7-year National Criminal History Search (County searches)
  • National Sex Offender Search
  • FACIS Level 3 search (includes OFAC)
  • EPLS/GSA/SAM and HHS/OIG searches (annual requirement)
• Education verification: Diploma or official transcripts required
• Physical exam within 1 year of start, updated annually
• Immunizations and health screenings including:

  • Negative TB test (QGold) within 1 year
  • Tetanus/Diphtheria/Tdap (declination accepted)
  • Measles, Mumps, Rubella (positive titer or proof of vaccination)
  • Hepatitis B (titer, declination, or proof of vaccine series)
  • Varicella (positive titer or proof of vaccination)
  • Seasonal Flu vaccination or signed declination (seasonal requirement)
  • COVID-19 vaccine received after 9/12/2023 or declination form
• Drug screening: 9-panel drug screen within 12 months (Aya requirement)
• Fit mask testing (N95) within 1 year for clinical staff
• Completion of facility-specific onboarding documents and training modules, including:

  • HIPAA training
  • Harassment prevention training (CA mandated)
  • UCLA-specific modules (e.g., C-ICARE, Code of Conduct, Radiation Hazard Awareness, Safe Patient Handling)
  • Confidentiality agreement
  • Access request forms and other facility-specific paperwork

Additional Requirements

• References: 2 required, supervisor level or above, with at least one reference within the last year
• Employment application covering last 7 years with explanations for gaps over 30 days
• Work history verification for last 5 years
• Skills checklist and competency exams as applicable

Notes

• Prior travel experience is not required for this position.
• Floating to sister facilities may be required based on client needs.
• Weekend and on-call shifts are not required.
