Job Description & Requirements

Minimum Qualifications

  • Valid LPN / LVN license in Illinois
  • Minimum 1 year of Long Term Care experience within the last year
  • Medical Surgical (Med Surg) experience
  • BLS certification issued by the American Heart Association

Work Schedule & Conditions

  • Guaranteed 40 hours per week.
  • Weekend Requirement: Every other weekend (2 weekends per month).
  • Return to Office (RTO): Maximum 5 days per month, no more than 2 consecutive days; no RTO on holidays or weekends.
  • Mandatory float: All clinicians must float within their scope to meet facility needs.

Certifications & Competencies Required

  • Current CPR certification.
  • Medication Test/Pharmacology Competency Exam (required for starts on/after 4/14/24, must be completed within 12 months of start).
  • Basic Life Support (BLS) - AHA only.
  • Other certifications may be required depending on unit assignment (e.g., ACLS, PALS, NRP) but not specifically required for LVN/LPN in this posting.

Health & Background Requirements

  • Background Check within 12 months covering all locations lived/worked/schooled in past 7 years.
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start.
  • Physical Exam within 12 months, signed by PA, NP, DO, or MD.
  • Fit Mask test within 1 year of start (N95 or equivalent), required annually.
  • Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination.
  • Negative TB test (QGold or TSPOT) within 12 months, updated annually.
  • Immunizations: Measles, Mumps, Rubella (2 MMR vaccines or positive titer), Hepatitis B (series, titer, or declination), Varicella (2 vaccines or positive titer).
  • Drug Testing: Negative 9-panel drug screen within 90 days of start, updated annually.
  • Seasonal Flu Vaccination or approved medical/religious exemption required during flu season (10/01 - 04/01).
