Job Opportunity: Case Manager (Registered Nurse)

Location: Crescent City, California
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Coordinate utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation.
  • Manage acute patient populations across the care continuum to provide coordinated and integrated care aimed at preventing unnecessary admissions or readmissions.
  • Provide discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department.
  • Promote patient wellness, improve care outcomes, ensure efficient utilization of health services, and minimize denials of payment among patients with complex health needs.
  • Work schedule: Monday to Friday, 8:00 AM to 4:30 PM (40 hours/week).

Required Qualifications

  • Valid Registered Nurse (RN) license in California.
  • Minimum of 1 year of experience in Acute Care Case Management, with experience within the last year.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Certifications & Competencies (as applicable)

  • Advanced Cardiac Life Support (ACLS) – verify if required by position.
  • Pediatric Advanced Life Support (PALS) – verify if required by position.
  • Neonatal Resuscitation Procedure (NRP) – verify if required by position.
  • Trauma Nursing Core Course (TNCC) – verify if required by position.
  • Electronic Fetal Monitoring (EFM) – typically for Labor & Delivery roles; verify if required.
  • Crisis Prevention Institute (CPI) – verify if required by position.
  • Medication Test/Pharmacology Competency Exam – required as applicable.
  • Dysrhythmia/EKG Competency Exam – may be required for some telemetry units; verify applicability.
  • Pass/Fail Testing (EKG/Medication) – ICU and Med/Surg/Telemetry RNs at certain locations may be required to take an onsite EKG exam after start.

Background & Screening Requirements

  • 7-year background check including Social Security trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening.
  • EPLS/GSA/SAM, HHS/OIG, OFAC Sanctions List, Medi-Cal Suspended and Ineligible Provider List searches as applicable.
  • Education verification required; Bachelor of Science in Nursing preferred.
  • Employment application must include full 7 years of work history with no gaps; employment verification required for up to 7 years.
  • Two professional references required within 3 years for clinical candidates.

Health & Immunization Requirements

  • Physical exam within 1 year of start, including clearance for work.
  • Fit mask test within 1 year, renewed annually (exceptions apply).
  • Adult Tetanus/Diphtheria/Tdap vaccine within 10 years or lifetime TDAP with booster within 10 years.
  • Negative TB test (QGold or TSPOT within 90 days) or 2-step PPD; annual renewal required.
  • Immunity or vaccination for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B (series or signed declination)
    • Varicella
  • Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per CA law).
  • Seasonal flu vaccination or signed declination during flu season (Nov 1 - Apr 30).
  • Color blindness exam if applicable (in-person exam required).
  • Respiratory safety clearance prior to start, renewed annually (exceptions apply).

Facility-Specific Requirements

  • Review and sign facility policies including Standards of Business Conduct, Vendor Training, Code of Conduct, Overpayment Refund Policy, and General Regulatory Orientation prior to start.
  • Complete facility-specific attestations such as Conditional Offer of Employment and Religious Accommodation forms if applicable.
  • Complete Epic Questionnaire (RN only) for candidates starting 11/3/2025 and onward.
  • Review parking and dress code policies prior to start.

Work Schedule

  • Day shift, Monday through Friday, 8:00 AM to 4:30 PM (5x8 hour shifts).
