Job Opportunity: Case Manager (Registered Nurse)
Location: Crescent City, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Coordinate utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation.
- Manage acute patient populations across the care continuum to provide coordinated and integrated care aimed at preventing unnecessary admissions or readmissions.
- Provide discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department.
- Promote patient wellness, improve care outcomes, ensure efficient utilization of health services, and minimize denials of payment among patients with complex health needs.
- Work schedule: Monday to Friday, 8:00 AM to 4:30 PM (40 hours/week).
Required Qualifications
- Valid Registered Nurse (RN) license in California.
- Minimum of 1 year of experience in Acute Care Case Management, with experience within the last year.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Certifications & Competencies (as applicable)
- Advanced Cardiac Life Support (ACLS) – verify if required by position.
- Pediatric Advanced Life Support (PALS) – verify if required by position.
- Neonatal Resuscitation Procedure (NRP) – verify if required by position.
- Trauma Nursing Core Course (TNCC) – verify if required by position.
- Electronic Fetal Monitoring (EFM) – typically for Labor & Delivery roles; verify if required.
- Crisis Prevention Institute (CPI) – verify if required by position.
- Medication Test/Pharmacology Competency Exam – required as applicable.
- Dysrhythmia/EKG Competency Exam – may be required for some telemetry units; verify applicability.
- Pass/Fail Testing (EKG/Medication) – ICU and Med/Surg/Telemetry RNs at certain locations may be required to take an onsite EKG exam after start.
Background & Screening Requirements
- 7-year background check including Social Security trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening.
- EPLS/GSA/SAM, HHS/OIG, OFAC Sanctions List, Medi-Cal Suspended and Ineligible Provider List searches as applicable.
- Education verification required; Bachelor of Science in Nursing preferred.
- Employment application must include full 7 years of work history with no gaps; employment verification required for up to 7 years.
- Two professional references required within 3 years for clinical candidates.
Health & Immunization Requirements
- Physical exam within 1 year of start, including clearance for work.
- Fit mask test within 1 year, renewed annually (exceptions apply).
- Adult Tetanus/Diphtheria/Tdap vaccine within 10 years or lifetime TDAP with booster within 10 years.
- Negative TB test (QGold or TSPOT within 90 days) or 2-step PPD; annual renewal required.
- Immunity or vaccination for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (series or signed declination)
- Varicella
- Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per CA law).
- Seasonal flu vaccination or signed declination during flu season (Nov 1 - Apr 30).
- Color blindness exam if applicable (in-person exam required).
- Respiratory safety clearance prior to start, renewed annually (exceptions apply).
Facility-Specific Requirements
- Review and sign facility policies including Standards of Business Conduct, Vendor Training, Code of Conduct, Overpayment Refund Policy, and General Regulatory Orientation prior to start.
- Complete facility-specific attestations such as Conditional Offer of Employment and Religious Accommodation forms if applicable.
- Complete Epic Questionnaire (RN only) for candidates starting 11/3/2025 and onward.
- Review parking and dress code policies prior to start.
Work Schedule
- Day shift, Monday through Friday, 8:00 AM to 4:30 PM (5x8 hour shifts).
