Required Qualifications

Valid Physical Therapist license in Iowa
Physical Therapist (1 year of experience)
Physical Therapist experience within the last year
BLS - Basic Life Support issued by American Heart Association or RQI
Experience with Epic

Additional Notes

Annual physical, fit test, TB questionnaire, and core competency exams must be kept current during the assignment
Hepatitis B documentation may be satisfied by a completed vaccine series, positive IgG titer, or declination form on file
Measles, mumps, rubella, and varicella immunity can be shown by positive IgG titers or documented vaccine series
Flu vaccination or approved exemption status must be on file during flu season
COVID-19 vaccination is preferred only
AHA BLS is required for all nursing and allied positions
