Associate of Science in Nursing / Associate Degree in Nursing
Hospice (1 year of experience)
Hospice experience within the last year
One completed travel assignment
BLS issued by American Heart Association or American Red Cross
Experience with OASIS
Experience with Homecare Homebase
Driver’s license/state ID
Background check within 1 year of start, covering all locations lived/worked/in school, and updated annually
EPLS/GSA/SAM required prior to facility start
HHS/OIG required prior to facility start
State license verification prior to facility start and upon expiration
NURSYS verification prior to facility start
Medication Test/Pharmacology Competency Exam
Competency Exam
Photo for badge within 72 hours of signing
Copy of auto insurance
Physical exam within the last 12 months of start
Negative TB or Chest X-Ray
TB questionnaire
Rubeola (Measles)
Mumps
