Overview

• Travel/Contract role in Home Health
• Start date: ASAP
• 8-hour day shift
• Guaranteed 36 hours weekly
• First-time travelers accepted
• Locals accepted
• On-call not required

Location / Coverage

• Home Health coverage areas include Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington, Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont
• May visit up to 6 patients per day
• Occasional maximum radius of 50+ miles
• Weekend visits may be picked up to meet productivity

Required Qualifications

• Valid Physical Therapy Assistant license in California
• Physical Therapy Assistant (1 year of experience)
• Physical Therapy Assistant experience within the last year
• BLS issued by American Heart Association or American Red Cross
• Experience in a Home Health setting

Preferred Qualifications

• 1 year of Home Health experience

Assignment Details

• Case load: Home Health
• 1:1 ratio
• 27 productivity points standard
• Traveler must see 5–6 patients per day
• Guaranteed hours do not apply in the first 2 weeks for ramp, holiday weeks, and time off weeks
• Traveler is expected to have their own transportation
• Timely communication and documentation required
• Documentation tools are provided and must be returned

Compliance / Onboarding

• Compliance takes about 2 weeks
• Government-issued photo ID required
• Emergency contact form required prior to start
• I9 attestation required
• Background check required
• EPLS/GSA/SAM required annually
• HHS/OIG required annually
• ACLS required
• PALS preferred
• National tech certification verification required as applicable
• State license verification required
• Medication test/pharmacology competency exam required as applicable
• Competency exam required as applicable
• Auto insurance required and must remain current throughout the assignment
• COVID declination form required if not vaccinated
• Employee data and emergency contact form required prior to start
• Arbitration agreement form required prior to start
• Confidential information agreement required prior to start
• Auto registration required and must remain current throughout the assignment
• Physical exam within 1 year of start required
• Tdap within 10 years of start or annual declination required
• Negative TB or chest X-ray within 1 year of start required
• Measles, mumps, rubella, varicella, hepatitis B, drug screen, and flu documentation requirements apply
• 2 professional references required within 3 years of start
• Skills checklist required annually
• 1 year of employment verification required
