Job Opportunity: Behavioral Health Nurse (LVN / LPN) - Travel/Contract
Location: Worcester, Massachusetts
Contract Length: 13 weeks
Position Overview
- Flexible schedule with potential changes in hours.
- Must be able to start on May 4.
- Ability and willingness to travel within a 35-mile radius to multiple locations daily (advanced notice provided).
- May require travel/transport duties between sites.
- Guaranteed 40 hours per week.
- Shift options:
- Day shift, 5 days a week, 8 hours per day (7:00 AM – 3:00 PM)
- Day shift, 4 days a week, 10 hours per day (7:00 AM – 5:30 PM)
Required Qualifications
- Valid LPN / LVN license in Massachusetts.
- Minimum of 1 year recent Behavioral Health experience within the last year.
- Current Basic Life Support (BLS) certification issued by the American Heart Association.
Key Responsibilities
- Deliver and manage client care using the Nursing Process from admission through discharge, appropriate to client developmental age.
- Perform client assessments and reassessments per standard procedures.
- Collaborate with interdisciplinary teams to develop and coordinate individualized care and discharge plans.
- Supervise, coordinate, and evaluate nursing care within the clinical setting.
- Delegate tasks appropriately based on competency levels.
- Document client care accurately and timely.
- Provide education to clients, families, and significant others, documenting accordingly.
- Administer medications, treatments, and therapies following policies and procedures.
- Assist licensed healthcare providers with treatments, procedures, and special tests as directed.
- Participate in professional development and assist in staff education.
- Engage in performance improvement initiatives and apply quality improvement in daily operations.
- Maintain professional ethics, conduct, and boundaries.
- Adhere to nursing standards recommended by the American Nurses Association and facility policies.
Compliance and Documentation Requirements
- Valid driver’s license or state ID.
- Massachusetts Statewide iCORI clearance and background checks including 7-year criminal history, SSN trace, national criminal search, and education verification.
- Education verification of highest completed education (transcript acceptable if education verification is unperformable).
- Physical exam within 12 months prior to start, including physician statement of fitness to work without restrictions.
- Drug testing within 30 days prior to start.
- Immunizations or valid declinations for:
- COVID-19 (most recent vaccine or declination)
- Influenza (seasonal vaccine or declination)
- Tetanus/Diphtheria/Tdap
- Measles (Rubeola), Mumps, Rubella
- Hepatitis B (vaccination series, reactive titer, or declination)
- Varicella (vaccination, positive titer, history, or declination)
- Tuberculosis screening: Two PPD tests (one within 90 days, one within 1 year) or QuantiFERON/T-Spot within 90 days; chest X-ray and TB questionnaire if positive.
- NURSYS license verification within 30 days prior to start.
- Medication and pharmacology competency exams required prior to start.
- Pass/fail medication exam required on first day of orientation with minimum 80% score.
- References: Two professional references within the last three years.
- Employment application documenting 7 years of employment history with explanations for gaps over 30 days.
- Skills checklist completed prior to start and updated annually as applicable.
Additional Notes
- Preference given to candidates with no Return to Office (RTO) restrictions.
- Candidates must be willing to work flexible hours and locations as needed.
- Prior travel experience is not explicitly required.
