Position Details

Job Description

• Join a growing Per Diem pool.
• Must be efficient with tracheostomy care and ventilators.
• Minimum compliance requirements must be met prior to starting.
• Full compliance profile must be completed within two weeks after start date.

Required Credentials & Compliance (Must be completed prior to or shortly after start)

• Licenses & Certifications:

  • Valid professional nursing license with primary source verification.
  • BLS (American Heart Association or Red Cross)
  • ACLS (American Heart Association or Red Cross)
  • PALS/NRP as required
• Vaccinations & Health Requirements:

  • COVID-19 Vaccine: 2 doses plus booster or declination completed.
  • MMR: Proof of 2 vaccines or positive titer.
  • Varicella: Vaccination or titer showing immunity.
  • Influenza Vaccine: Required for Nov 1, 2025 - Mar 31, 2026 (declinations accepted except at certain locations).
  • Hepatitis B: Declinations accepted.
  • Tdap: Declinations accepted.
  • TB Test: Within one year of start date.
  • Physical/Health Statement: Annually, must state clear to work.
• Testing & Screening:

  • Drug Screen: Minimum 10-panel test including THC saliva test (specific to California).
  • Background Check: National criminal background check within one year of start.
• Other Requirements:

  • Driver’s License, State ID, or Passport.
  • Prime Core Competencies Exam: Must score 80% or higher (attestation accepted).
  • Prime Skills Checklist.
  • Prime OIG exclusion search.
  • Prime SAM/EPLS exclusion search.
  • Prime Agency Attestation (completed after all credentials uploaded).
  • Signed Job Description.
  • Resume and 2 professional references (1 must be from a manager or direct supervisor).

Optional but Recommended

• Annual Respirator Fit Test (may be required prior to start depending on location).
• Positive PPD with negative chest X-ray (every 4 years if applicable).
