Job Opportunity: Respiratory Therapist

Location: Des Plaines, Illinois
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• Night shift
• Works every other weekend and every other holiday
• Flexibility with scheduling is a must (no block schedules)

Required Credentials & Certifications

• Driver's License, State ID, or Passport (required for onboarding)
• BLS (Basic Life Support) – American Heart Association or Red Cross
• PALS (Pediatric Advanced Life Support) – American Heart Association or Red Cross
• ACLS (Advanced Cardiovascular Life Support) – American Heart Association or Red Cross
• NRP (Neonatal Resuscitation Program) – American Heart Association, Red Cross, or NALS (for NICU, L&D, Postpartum RNs)
• Professional License/Certificate Verification – Primary source verification within 30 days of start, updated upon expiration
• COVID-19 Vaccine – 2 vaccines plus booster or completed declination
• MMR (Measles, Mumps, Rubella) – Proof of 2 vaccines or positive titer showing immunity
• Hepatitis B – Vaccination required (declinations not accepted at some locations)
• Influenza Vaccine – Required during flu season or declination completed
• TB Test – Within one year of start date
• Varicella – Vaccination or titer showing immunity
• Tdap – Required (declinations not accepted at some locations)
• Annual TB Questionnaire
• Fit Test – Annual respirator fit test; must be completed prior to start for Illinois region
• DCI/BVP (Workplace Violence Prevention Training) – Required; can be completed via Relias or similar platforms

Additional Requirements

• Prime Resume – Up-to-date and accurate work history (required at offer acceptance)
• References – Two required, including one from a manager or direct supervisor
• Prime Skills Checklist – Required for submission
• Prime Core Competencies Exam – Must score 80% or higher
• Prime Criminal Background Check – National check within one year of start date
• Social Security Card or Passport – Required for onboarding
• Physical/Health Statement – Annual, must state clear to work
• Prime Badge Photo – Clear headshot with plain background (JPEG)
• Prime Form I-9 (Employment Eligibility Verification) – Agency attestation accepted
• Prime Job Description – Provided by agency and signed by employee
• Onboarding Education Attestation – Must be completed and submitted prior to clearance to start
• Essentials Onboarding Packet and Orientation Packet – Required for onboarding
