Surgical Technician Job Opportunity

Location: Dubuque, Iowa
Employment Type: Long-Term Assignment (13 weeks)
Shift: 06:30 AM – 5:00 PM (start time may vary between 06:30-17:00 or 11:00-21:30)

Job Details

• On-call responsibilities:

  • One weeknight per week from end of shift until 7:00 AM
  • Every 4th weekend from Friday end of shift through Monday 7:00 AM
  • Holidays require either a work shift or on-call duty (hours vary; Labor Day included)

Required Qualifications

• Certified Surgical Technologist (CST) or NBSTSA certification
• BLS certification issued by the American Heart Association
• Minimum of 1 year of CVOR Technologist experience within the last year
• Surgical Technician experience

Health and Compliance Requirements

• COVID-19 Vaccine: Proof of booster by September 2025, or declination/exemption with agency letterhead (declination not accepted in CA or IL)
• MMR: Proof of 2 vaccinations with dates or IgG titer lab report
• Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later); titers not accepted
• Influenza Vaccine: Optional; proof or declination accepted (declination not accepted in CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray within last year or 5 years depending on region
• Varicella: Proof of 1 vaccination or IgG titer lab report
• Tdap: Current, non-expired vaccine required; declination accepted if no pediatric interaction
• Fit Test: Required for Iowa locations prior to start (Dubuque included)
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work
• Drug Screen: 10-panel drug screen completed within 30 days of start (preferred within 14 days), must include MRO signature; no THC allowed
• Color Vision Test: Required

Professional and Background Requirements

• Resume and Skills Checklist (within one year)
• Two professional references
• Confidentiality and Security Agreement signed
• Conflict of Interest Disclosure Form signed
• Criminal Background Check: Federal, nationwide, county (last 7 years), including SSN verification, dated within 30 days of start
• Education Verification: Highest academic degree or diploma/GED verified by third party screening
• EPLS (Excluded Parties List System) check dated within 30 days
• OIG (Office of Inspector General) exclusion check dated within 30 days
• Integrity and Audit Services Form signed shortly before start
• CORE Exam (Joint Commission Testing) within one year
• NSO (National Sex Offender) check dated within 30 days, including aliases and statewide checks for MI
• System Access Form completed and uploaded one week before start
• Specialty Specific Competency Testing: Completed within one year
• Child and Adult Dependent Abuse Training (Iowa only)
• Code of Conduct Form signed
• I9 form completed by agency
• Iowa Sing Report: Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days
• Non-Employee Upload Form pages 1-3 and 28 completed and uploaded

Additional Notes

• Agency will provide the Skills Checklist for clinical contractors
• All documentation must be uploaded to the vendor management system prior to onboarding as specified

This position requires a Surgical Technician with current certifications and compliance with extensive health, background, and professional credentialing standards. The role includes variable shifts with on-call duties and holiday coverage.
