Physical Therapist (Rehabilitation Therapy) – Contract Position

Location: Grand Junction, Colorado
Employment Type: Contract (13 weeks)
Shift: Full-Time, 40 hours per week

Job Summary

Provide diagnostic and therapeutic physical therapy services for patients of all ages in both acute care and outpatient settings. Responsibilities include patient evaluation, establishing and implementing plans of care, assessing treatment effectiveness, modifying treatments as needed, and educating patients and caregivers. Collaborate with multidisciplinary teams to support patient independence and maintain quality care.

Key Responsibilities

• Coordinate and direct patient care services across the lifespan (adult and pediatric patients).
• Perform patient evaluations and assessments to develop and implement individualized treatment plans.
• Monitor and adjust treatments to ensure achievement of therapy goals.
• Provide patient, caregiver, and staff education using effective teaching methods.
• Maintain timely, accurate, and professional documentation in compliance with regulatory and professional standards.
• Understand and apply knowledge of treatment costs, billing, coding, and procedural planning to support quality and efficiency.
• Submit accurate billing and coding information for services rendered.

Required Qualifications

• Valid Physical Therapist license in Colorado
• Minimum of 2 years of experience as a Physical Therapist
• Physical Therapist experience within the last year
