Registered Nurse Diabetes Educator
Location: Rockford, Illinois
Employment Type: Contract (13 weeks)
Shift: Monday to Friday, Day Shift (40 hours/week)
Job Summary
Provide compassionate, culturally sensitive, holistic nursing care focused on diabetes education for patients and families living with diabetes mellitus. Collaborate with a multidisciplinary diabetes management team to develop and execute individualized patient care plans. May float to various units based on clinical competency and departmental needs.
Essential Duties and Responsibilities
- Utilize the nursing process and collaborate with interdisciplinary team members in patient care.
- Administer prescribed medications and treatments following nursing standards, policies, and state laws.
- Document care according to standards.
- Develop, initiate, and execute individualized patient care plans, including patient and family education.
- Participate in department/unit performance improvement and shared governance activities.
- Provide clinical expertise and guidance to other units/departments supporting patient care continuity.
- Apply critical thinking to ensure patient safety and experience; escalate issues appropriately.
- Act as a resource for staff regarding patient education programs and materials.
- Advocate for patients addressing clinical, psychosocial, and financial issues.
Required Qualifications
- Valid RN license in Illinois
- Associate of Science in Nursing / Associate Degree in Nursing
- CDCES
- Diabetic Educator experience
- Educator (2 years of experience)
- Educator experience within the last year
Additional Information
- Work schedule is Monday through Friday, day shift.
- May include variable shifts, weekends, holidays, and/or on-call hours as needed.
- Ability to float to various units based on clinical competency.
