Job Description
- Provide a full range of professional nursing services to clients of all ages in a home health and hospice setting
- Responsibilities include admission, evaluation, and care plan development
- Collaborate with a team of homecare professionals and paraprofessionals to deliver coordinated care
Required Qualifications
- Valid RN license in Vermont
- Minimum 1 year of home health nursing experience within the last year
- BLS certification
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

- Work area includes Caledonia and southern Essex counties
