Unit Information

• Unit: Case Management (Unit 917)
• Shift: Days, typically 8:00 AM – 4:30 PM, Monday – Friday, with weekends as needed
• Unit Size: Approximately 20 beds per unit
• Staffing:

  • Social Workers (SW) ratio: 1:40
  • Registered Nurses (RN) ratio: 1:20
• Work Environment: Multiple floors, Case Management office located separately
• Equipment: Mobile devices, computers
• EMR Systems: Meditech, Midas, Navihealth

Required Skills and Experience

• Behavioral Health experience, specifically:

  • Suicide risk assessments
  • Substance use assessments (SBIRT)
• Discharge planning expertise

Preferred Skills

• Emergency Department (ED) experience (helpful but not required)
• Knowledge of Palliative Care and Goals of Care (GOC) conversations
