Job Description

• Conduct initial assessments for patients on legal holds (e.g., 5150, 5585, 5250) including evaluation of mental status, living situation, support system, drug and mental health history, physical functional status, financial status, and discharge planning needs.
• Complete Social Determinants of Health (SDOH) assessments for every admitted patient.
• Manage ongoing care and follow-up recommendations for psychiatric patients under legal hold.
• Communicate regularly with nursing management and unit social workers/case managers regarding patient status, legal hold expiration, and discharge planning.
• Maintain contact with external agencies and families for collateral information and discharge arrangements (non-medical).
• Monitor patients daily or multiple times per week depending on legal hold status.
• Assist with transfer to appropriate psychiatric or legal facilities when medically cleared, and develop alternative discharge plans when necessary.
• Monitor potential conservatorship needs for patients under legal hold, ensuring timely referrals to Public Guardian when appropriate.
• Collaborate with psychiatrists and medical social workers regarding patient care and legal status transitions.

Required Qualifications

• Valid Master of Social Work license in California.
• Master of Social Work degree with at least 1 year of experience.
• Recent experience as a social worker within the last year.
• Experience in social work practice.
