Background and Compliance Requirements

  • Background Check covering last 7 years, including:
    • Social Security Address Check (last 15 years)
    • National Criminal Search
    • Sex Offender Search
    • Education Verification
    • International Searches if applicable
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days prior to start
  • State-specific Medi-Cal Exclusion Search (California) within 30 days prior to start
  • Education verification required
  • Complete 15 years of work history on application (including locations and gaps)

Health and Immunization Requirements

  • Physical exam within 12 months clearing employee to work without restrictions
  • Fit Mask test (specific approved masks; onsite testing possible with approval)
  • Tdap vaccine or declination within 10 years (Td not accepted)
  • Negative TB test or chest X-ray per facility protocol
  • TB Questionnaire (required for all candidates)
  • Immunity or vaccination records for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (vaccination series, titer, or declination)
    • Varicella (vaccination or titer; history of disease not accepted)
    • Hepatitis C (titer accepted)
  • Negative 10-panel drug screen within 30 days of start (no instant tests)
  • Annual flu vaccination or facility-specific declination (mask required if declined)
  • Seasonal COVID vaccination or facility-specific declination (mask required if declined)
  • Medical Release Form (annual)

  • Must review and sign multiple facility-specific documents prior to start, including:
    • Orientation Packet Acknowledgment
    • Float Policy (for ICU/Critical Care)
    • Timekeeping Acknowledgment
    • Acknowledgement of Responsibility
    • Standard of Care Acknowledgement (for certain units)
    • TJC Attestation (updated annually)

    • Parking Regulations
  • Required to complete assigned online modules via facility LMS (WorkRamp) with passing score >80%
  • Drug Diversion Module required for all RNs and LVNs prior to start

References and Work History

  • Two professional references required (manager, supervisor, Charge RN, or HR)
