Required Certifications & Licenses

• BLS (Basic Life Support) - AHA or ARC, current
• ACLS (Advanced Cardiac Life Support) - AHA or ARC, current
• NIHSS (NIH Stroke Scale Certification) - Annual update required
• Valid State RN License (verified within 30 days prior to start)
• Driver’s License and Utility Bill (utility bill must be in candidate’s name, with matching address outside 75-mile radius)

Additional Certifications (Unit Dependent)

• PALS (Pediatric Advanced Life Support)
• NRP (Neonatal Resuscitation Program)
• TNCC (Trauma Nursing Core Course)
• EFM (Electronic Fetal Monitoring)
• CPI (Crisis Prevention Institute) for Behavioral Health Units

Competency Requirements

• Medication Test/Pharmacology Competency Exam (annual, 100% passing score)
• Color Blind Testing (within 60 days prior to start)
• Unit-specific Competency Exam (annual, minimum 80% passing score)
• Dysrhythmia/EKG Competency Exam (annual, minimum 90% passing score)
• Core Parts I, II, and III Competency Exams (annual, minimum 80% passing score)
• Behavioral Competency Exam (for behavioral health roles, performed onsite)

Health & Immunization Requirements

• Physical Exam/Statement of Good Health (within 1 year)
• Fit Mask Testing (completed onsite first day)
• Tetanus/Diphtheria/Tdap (Tdap mandatory for certain units)
• Negative TB test or Chest X-Ray (within 12 months)
• TB Questionnaire (annual)
• Measles (Rubeola), Mumps, Rubella - 2 vaccines or positive titer, boosters if low titer
• Hepatitis B - vaccine series, positive titer, or declination (annual if declination)
• Varicella - 2 vaccines, titer, or history of chickenpox
• Drug Testing - Negative 10-panel including marijuana (within 30 days prior to start)
• Hepatitis A (required for Pediatric/PICU and certain campuses)
• Seasonal Flu Vaccination (required annually for new starts)
• COVID Vaccination - Strongly preferred; declinations accepted with agency form

Background & Verification Requirements

• 7-year criminal background check (within 30 days of start) including multiple databases
• EPLS/GSA/SAM and OFAC Sanctions List Search (updated monthly)
• Education Verification (3rd party verification of highest degree)
• NURSYS Verification (RN license verification within 30 days prior to start)
• Work History Verification (10 years, including international)
• References: 2 direct supervisor references (one within 1 year, one within 3 years)
• Current and updated resume covering last 7 years, with explanations for gaps >30 days
• Skills Checklist (within 1 year, updated annually)

Facility Specific Requirements

• Patient Care Orientation Class (Skills) mandatory for all RNs
• EPIC training required if no EPIC experience within last 6 months
• Fit Testing for N95 masks completed onsite within 1 year
• Vehicle Information form must be completed prior to start
• Completion of various policy acknowledgments and agreements prior to start (Ethical Business Conduct, Drug and Alcohol-Free Workplace, Information Access, Oklahoma Offender Registration Act, etc.)
• Fingerprinting and Oklahoma Screen clearance required prior to start

Notes

• Candidates must submit a valid driver’s license and a utility bill with their application.
• No more than 5 days of Return to Office (RTO) will be accepted.
• Candidates must live outside a 75-mile radius from the facility.
• First-time travelers are not accepted for this position.
