Job Opportunity: Interventional Radiology (Cath Lab Tech/IR Tech)

Location: Oak Park, Illinois

Specialty

• Required Experience: Minimum 1 year in specialty (Radiology/Cardiology)
• Illinois License required or willingness to obtain

Certifications & Competencies

• ACLS (American Heart Association) required for Cath Lab
• BLS (American Heart Support) required
• Tech Certification Verification: AART and IEMA required for Radiology
• Medication/Pharmacology Competency Exam required for all RNs/LPNs with minimum 80% passing score
• Orientation Safety Manual Acknowledgement required
• State License Primary Source Verification within 30 days of start
• NURSYS Verification (for RN, LPN/LVN, AP) within 30 days of start

Background & Compliance Requirements

• Background Check within 30 days of start covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
• Fingerprinting: Illinois Health Care Worker Registry Search required for patient-facing candidates without Illinois state license
• IL Mandatory Reporting Training required for all clinical patient-facing/in-person positions (valid for 3 years)

Health & Immunization Requirements

• Physical Exam within 1 year of start, confirming fitness to work without restrictions
• Drug Testing: 9-panel drug screen within 90 days of start (excluding marijuana)
• Immunizations/Vaccines:

  • Tetanus/Diphtheria/Tdap (valid 10 years, TD not accepted)
  • Negative TB test or Chest X-Ray with appropriate documentation
  • Rubeola (Measles), Mumps, Rubella: 2 MMR vaccines or positive IgG titer
  • Hepatitis B Surface Antibody Titer required; vaccine series or declination form if negative
  • Varicella: 2 vaccines or positive IgG titer
  • Flu Seasonal Vaccination or approved exemption required for 2025-2026 season
• COVID-19 Vaccine: Not mandated but documentation encouraged

Documentation & Other Requirements

• Employment Application including 7 years of employment history and explanations for gaps >30 days
• Professional References: 2 within last 3 years (Aya Direct only)
• Facility-specific forms such as Hep B declination, TB screening questionnaire, flu exemption form, and mandated reporter forms
• Fit Mask completed on-site (documentation maintained by facility)
