Physical Therapist - Home Health
Location: Stockton, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Field position covering Central Valley area including Stockton and Modesto
- Home Health setting
- Schedule options:
- Monday to Friday with one weekend day on call per month
- Tuesday to Saturday
- Wednesday to Sunday
- Local candidates are welcome
- Home Health experience preferred; if no Home Health experience, outpatient experience is required as a minimum
- OASIS experience required; candidates without OASIS experience may be considered if they have strong Home Health experience
- Hours based on completed visits (25 visits = 40 hours); hours guaranteed if 25 or more visits are completed
- COVID exemptions permitted
- Mileage reimbursed at IRS standard rates
- Candidates with pending California license will be considered if application is submitted
Required Qualifications
- Experience in an Outpatient setting
- OASIS experience
- Valid California Physical Therapist license preferred (pending license acceptable if application submitted)
- Basic Life Support (BLS) certification (AHA Instructor or Provider acceptable)
- Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Procedure (NRP), and Electronic Fetal Monitoring (EFM) certifications as applicable
- Physical exam within 12 months of start
- Required immunizations and health screenings including:
- Tetanus/Diphtheria/Tdap (within past 10 years, no declinations accepted)
- Negative TB test or chest X-ray within 1 year (with additional requirements if past positive)
- Measles, Mumps, Rubella immunity documentation (for candidates born in 1957 or later)
- Hepatitis B titer or vaccine series (declination accepted if within 1 year)
- Varicella immunity or vaccination (declinations not accepted)
- Negative 9-panel drug screen within 1 year
- Seasonal flu vaccination or declination accepted
- COVID vaccination or approved medical/religious exemption
Additional Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Valid passport copy before start
- I9 attestation and employment application required
- Background checks including 7-year history, EPLS/GSA/SAM, and HHS/OIG checks (updated annually)
- Photo for badge (passport style)
- Personal or auto insurance documentation
- Auto registration documentation
- Completion of facility-specific training and policy acknowledgments prior to start
- Professional references (2 required)
- Skills checklist updated annually
- Competency exams as applicable with minimum passing scores
