Job Summary

Seeking a Physical Therapist for a home health travel contract position. The role involves providing therapy and rehabilitation services in a demanding work environment, requiring strong critical thinking, problem-solving, and independent judgment skills.

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 2 years of experience as a Physical Therapist, including 2 years of experience in PT Home Health
• Physical Therapist experience within the last year
• Basic Life Support (BLS) certification (AHA or ARC)

Background and Compliance Requirements

• 7-year background check including SSN trace, county searches, sex offender search, education verification, motor vehicle report, and FACIS Level 3 screening
• Drug screening: Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids per California law)
• Physical exam within 1 year of start
• Fit mask test within 1 year, renewed annually (with accommodations for religious exemptions or facial hair)
• Immunizations and titers required:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test or chest x-ray (annual renewal required)
  • Measles, Mumps, Rubella (MMR) immunity or vaccination
  • Hepatitis B vaccination series or declination
  • Varicella immunity or vaccination
  • Seasonal flu vaccination or signed declination during flu season (Nov-April)
• Respiratory safety clearance required annually (not required for some non-clinical roles)
• Education verification and transcript/diploma submission as needed
• Employment history verification covering 7 years with no gaps over 30 days
• References: 2 professional references within 3 years required (Aya Healthcare requirement)

Certifications (as applicable)

• ACLS (Advanced Cardiac Life Support) – verify by position
• PALS (Pediatric Advanced Life Support) – verify by position
• NRP (Neonatal Resuscitation Program) – verify by position
• Other certifications may be required depending on assignment specifics
