Location: Santa Rosa, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
Seeking a Physical Therapist for a home health travel contract position. The role involves providing therapy and rehabilitation services in a demanding work environment, requiring strong critical thinking, problem-solving, and independent judgment skills.
Required Qualifications
- Valid Physical Therapist license in California
- Minimum 2 years of experience as a Physical Therapist, including 2 years of experience in PT Home Health
- Physical Therapist experience within the last year
- Basic Life Support (BLS) certification (AHA or ARC)
Background and Compliance Requirements
- 7-year background check including SSN trace, county searches, sex offender search, education verification, motor vehicle report, and FACIS Level 3 screening
- Drug screening: Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids per California law)
- Physical exam within 1 year of start
- Fit mask test within 1 year, renewed annually (with accommodations for religious exemptions or facial hair)
- Immunizations and titers required:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Negative TB test or chest x-ray (annual renewal required)
- Measles, Mumps, Rubella (MMR) immunity or vaccination
- Hepatitis B vaccination series or declination
- Varicella immunity or vaccination
- Seasonal flu vaccination or signed declination during flu season (Nov-April)
- Respiratory safety clearance required annually (not required for some non-clinical roles)
- Education verification and transcript/diploma submission as needed
- Employment history verification covering 7 years with no gaps over 30 days
- References: 2 professional references within 3 years required (Aya Healthcare requirement)
Certifications (as applicable)
- ACLS (Advanced Cardiac Life Support) – verify by position
- PALS (Pediatric Advanced Life Support) – verify by position
- NRP (Neonatal Resuscitation Program) – verify by position
- Other certifications may be required depending on assignment specifics
