Physical Therapist - Home Health (Travel/Contract)
Location: San Jose, California

Job Description
- Provide physical therapy services primarily in home health settings, including wound care and geriatric patients.
- Patient visits focus on quality rather than quantity; minimum 23-24 visits per week required to qualify for a 40-hour guarantee.
- No weekend work required.
- On-call duties are discretionary but not required.
- May be asked to cover surrounding counties; territory details discussed during interview.
- Locals within a 50-mile radius are accepted.
Contract Length
- 4 to 6 months per contract.
- Less experienced clinicians must commit to 6 months (non-negotiable).
Required Qualifications
- Valid Physical Therapist license in California
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Experience in a Home Health setting
- BLS (Basic Life Support) certification
Work Environment & Expectations
- Dress code: Any color scrubs with closed-toe shoes.
- Charting must be completed onsite using specified electronic systems; no remote charting allowed.
- Free residential parking available.
- Mileage reimbursed from first to last patient visit (not to/from home).
- COVID-19 vaccination required; religious exemptions allowed.
- Flu vaccination required or declination form during flu season.
- Compliance documents must be completed before starting patient care; compliance processing may take 2-3 weeks.
- All required return-to-office (RTO) documents must be submitted with application; no additions accepted after offer.

Required Documentation & Compliance (Summary)
- Government-issued photo ID.
- Background checks including SS trace, county searches, and national databases within 1 year.
- Physical exam within 6 months confirming freedom from communicable diseases.
- Immunizations or proof of immunity for:
- Measles, Mumps, Rubella (MMR)
- Hepatitis B
- Varicella
- Tetanus/Diphtheria/Tdap (within 10 years or exemption)
- Negative TB test or chest X-ray within 1 year.
- 9-panel drug screen within 1 year.
- Seasonal flu vaccination or declination during flu season.
- COVID-19 vaccination or approved medical/religious exemption.
- Auto insurance card (if applicable).
- Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care.
Prior Travel Experience
- Not required; first-time travelers are welcome.
