Job Opportunity: Physical Therapist - Home Health

Location: Medford, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide skilled physical therapy services in a home health setting.
  • Work 8-hour day shifts (no 4x10 or 3x12 shifts available).
  • Patient ratio is 1:1; may visit up to 6 patients per day.
  • Productivity standard: 30 points per day.
  • Must have own transportation; occasional travel radius up to 50+ miles.
  • Charting system: EPIC/Oasis.
  • Facility requires primary series COVID vaccination plus one booster.
  • Attire: Any color scrubs.
  • Parking: Free.
  • Mileage reimbursed at IRS standard rates.
  • Locals within 50-75 miles considered local and accepted.

Required Qualifications

  • Valid Physical Therapist license in Oregon.
  • Minimum 1 year of Physical Therapist experience within the last year.
  • One completed travel assignment.
  • Experience in a Home Health setting.
  • Experience with OASIS documentation system.

Compliance & Documentation Requirements

  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO.
  • Background check within 45 days of start, covering all locations lived/worked in past 7 years.
  • Drug screen: 10 panel within 90 days of start (instant tests not accepted).
  • Immunizations and health documents required:
    • COVID vaccination or declination.
    • Flu vaccine during flu season or declination.
    • Tetanus/Diphtheria/Tdap within 10 years or declination.
    • Negative TB test or chest X-ray within 12 months, updated annually.
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer.
    • Varicella vaccination or positive titer.
    • Hepatitis B vaccination series or signed declination.
  • Respirator clearance required annually for travel candidates.
  • Fit mask testing required for travel candidates, updated annually.
  • References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years.
  • Employment application covering at least last 5 years required.

Additional Notes

  • Traveler is expected to complete required regulatory and specialty-specific training modules annually via the employer’s LMS platform.
  • Modules include system-wide regulatory requirements and specialty-specific content.
  • Physical exam and health documents must be updated annually as required.
  • No weekend or on-call shifts.
  • Locals accepted and considered at the same rate.
  • Must have reliable transportation due to travel between patient homes.
  • Facility cannot accommodate block schedules or extended shifts.
